Zusammenfassung
Die computerunterstützte Videokeratographie (computer-assisted videokeratography, CAVK) hat sich im letzten Jahrzehnt als wichtige Untersuchungsmethode im Bereich der Hornhautchirurgie fest etabliert, und ist bei Indikationsstellung, Qualitätssicherung und vor allem der Feststellung von Kontraindikationen, besonders in der refraktiven Hornhautchirurgie unentbehrlich geworden. Bislang fehlte es jedoch noch an Systemen, welche rasch, präzise und — vor allem auch intraoperativ — Untersuchungen am liegenden Patienten ermöglichen.
Die Anwendbarkeit des Keratron-Scout®-Systems im Operationssaal wurde bei verschiedenen Fragestellungen getestet: (a) unmittelbar vor und nach Implantation von intracornealen Ringsegmenten (INTACS®), (b) nach photorefraktiver Keratektomie (PRK), (c) nach Laser-Thermokeratoplastik (LTK), (d) bei schrittweiser selektiver Nahtentfernung nach perforierenden Keratoplastiken zur Reduktion eines Astigmatismus und (e) zur Minimierung des induzierten Astigmatismus nach Katarakt-Operationen.
Dieses neue System erlaubte erstmalig präzise Aussagen über Brechkraft und Krümmungsdaten der Hornhaut, oft bereits wenige Augenblicke nach Beendigung des jeweiligen refraktiven Eingriffs. Die Zentrierung bzw. das vorläufige Ergebnis des Eingriffs können somit schon intraoperativ überprüft und gegebenenfalls noch im selben Eingriff modifiziert werden. Die Reproduzierbarkeit der angefertigten Topographien kann mittels integrierter Software auf einfache Weise raschest überprüft werden.
Summary
Computer-assisted videokeratography (CAVK) has been proven to be an indispensable tool in the field of corneal and refractive surgery. Up to the present no commercial topographer was available that could reproducibly measure a supine patient using the Placido method. In this study the practicability of the new corneal topographer „Keratron Scout®“ was evaluated performing measurements (a) before and immediately after implantation of INTACS®, (b) after photorefractive keratectomy (PRK), (c) after diodelaserthermokeratoplasty and (d) cataract surgery. It was also used (e) as a „intraoperative guide“ for selective removal of interrupted sutures following penetrating keratoplasty.
Accurate and very reproducible topographic maps could be obtained within virtually seconds after the surgical procedure. These results could be used for making immediate adjustments in the OR and thereby — hopefully — improve of the final outcome.
For the first time a CAVK-system is available that can precisely measure the topography in a supine position under sterile surgical conditions.
