Zusammenfassung
Netzhautischämien, die dem Morbus Purtscher ähneln, treten bei 2% der Patienten mit akuter alkoholinduzierter Pankreatitis auf. Die Netzhaut eines 38jährigen Patienten mit chronischer Pankreatitis zeigte eine ausgeprägte Ischämie ohne Gefäßverschlüsse. Zusätzlich war der Vitamin B12-Spiegel erniedrigt. Während der Beobachtungszeit von 6 Monaten besserte sich das Sehvermögen trotz Substitution nur geringfügig, obwohl die Pankreatitis abklang. Neben dem Rezeptorschaden war ein ausgedehnter Nervenfaserschwund für die dauerhafte Funktionseinbuße verantwortlich.
Summary
2% of all patients with alcohol-induced pancreatitis develop visual disturbances presenting a retinal image similar to Purtscher’s retinopathy.
In a 38-year male Caucasian, suffering from chronic pancreatitis, acute retinal ischemia without vascular occlusion caused severe visual disturbances. Inspite of rapid improvement of the pancreatitis and the electroretinogram, the visual function did not recover due to severe loss of photoreceptor function and retinal nerve fibres. A lack of Vitamin B12 may have pronounced the ischemic damage of the optic nerve.
