Zusammenfassung
Hintergrund
Vor dem routinemäßigen Einsatz des Nidek AR 800 Autorefraktometer sollte die Zuverlässigkeit der objektiven Autorefraktometrie überprüft werden.
Material und Methoden
An 50 unselektierten konsekutiven Patienten unserer orthoptischen Abteilung wurde die Refraktion beider Augen mit dem Nidek AR 800 Autorefraktometer in Cyclopentolat-Zykloplegie bestimmt und mit einer Referenzmessung am Hartinger Koindizenzrefraktometer verglichen.
Ergebnisse
Wir fanden eine hohe Übereinstimmung des sphärischen Brechungsfehlers (r = 0,98 bzw. 0,97) und des sphärischen Äquivalents (r = 0,98 bzw. 0,97).
Der Vergleich der Zylinderstärke ergab einen Korrelationskoeffizienten von 0,93 bzw. 0,88. Die mittlere Abweichung der Zylinderachse betrug 11,7 Grad. Bei Zylinderstärken bis zu 0,5 cyl dpt war die Achsabweichung 15,5 Grad, bei höherem Astigmatismus 7,5 Grad.
Schlußfolgerungen
Die kurze Meßzeit und die damit möglichen Mehrfachmessungen, sowie das für Kinder interessante Fixationsobjekt (bunter Fesselballon am Ende einer Straße) ermöglichen eine zuverlässige und weitgehend stressfreie Messung mit dem Autorefraktometer.
Refraktionsbestimmungen sind mit dem Nidek AR 800 ab dem 3. Lebensjahr ausgezeichnet möglich.
Die Skiaskopie ist allerdings bei Kleinkindern und unkooperativen Patienten durch die automatisierte objektive Refraktometrie nicht zu ersetzen.

Background
Before using the Nidek AR 800 autorefractor for routine measurements, we tested the reliability of the results in comparison to manual refractometry.
Materials and Methods
Objective automated refraction using the Nidek AR 800 autorefractor was performed in both eyes of 50 non-selected consecutive patients of our strabismological service in cyclopentolate cycloplegia.
The data obtained were compared with the results of measurements of the same patients performed by Hartinger refractometer.
Results
We found a good correlation with respect to spherical power (r = 0.98, resp. 0.97) and spherical equivalent (r = 0.98, resp. 0.97) comparing both methods.
Estimating the cylinder power we found a correlation coefficient of 0.93 resp 0.88. The mean deviation of the cylinder axis was 11.7 degrees. If the cylinder power was 0.5 dpt or less the mean deviation of the axis was 15.5 degrees, in cylinder power larger than 0.5 dpt the mean axis deviation declined to 7.5 degrees.
Conclusions
The short duration of a single measurement and thus possible multiple measurements allow a reliable and almost stress-free examination using the autorefraktor. The fixation object is interesting for children and attracts their attention.
Objective refractometry with the Nidek AR 800 autorefractor can easily be performed in children three years of age and up.
However, automated objective refractometry is not able to replace skiascopy in small infants and non compliant patient as well as the patient in supine position.
