Zusammenfassung
Neben der Mitosehemmung der Betastrahlung ist eine ergänzende therapeutische Wirkung durch die Vasoobliteration bedingt, sodaß ein Einsatz der Brachytherapie bei primär gefäßassoziierten Erkrankungen sinnvoll erscheint.
Wir haben seit 1988 bei 2 Patienten jeweils beidbzw. einseitig eine progressive oberflächliche und tiefe zirkuläre Vaskularisation bei Zustand nach perforierender Keratoplastik, bei weiteren drei Patienten einen Gefäßpannus mit Vaskularisation der Hornhaut nach Laugenverätzung und ein Rezidiv eines auf die Hornhaut übergreifenden Bindehautpapilloms mit Strontium 90 bestrahlt. Die Einzelfraktion betrug jeweils 10 bzw. 15 Gy, die Gesamtdosis in Abhängigkeit von der Grunderkrankung zwischen 30 und 60 Gy. In allen Fällen reichte das Ergebnis von vollständiger Vasoobliteration bis zur Stagnation der Progression; im 1. Fall einer beidseitigen Vaskularisation nach Keratoplastik blieben beide Läppchen klar, das Papillomrezidiv bildete sich nach Strahlentherapie vollständig zurück. In keinem Fall konnte bis jetzt eine radiogene Komplikation beobachtet werden. Die Radiotherapie des vorderen Segments sollte jedoch nur solchen ausgesuchten Fällen vorbehalten bleiben, bei denen das konservative therapeutische Spektrum die Progression der Vaskularisation nicht verhindern konnte.
Bei einer Einzeldosis von 15 Gy sollte die Gesamtdosis 60 Gy nicht überschreiten, da gerade am vorderen Augensegment die Grenze zwischen therapeutischem Nutzen und radiogener Komplikation fließend ist.
Summary
As Strontium 90 irradiation prevents mitotic activity as well as obliteration of vessels brachytherapy seems to be useful in treatment of vascular diseases.
Since 1988 we have been treating 2 patients with progressive vascularisation of the corneal graft, 3 patients with recurrent vascular pannus and vascularisation after acid burn and one patient with a recurrent papilloma of the conjunctiva and cornea. The single dose was 15 Gy, the total dose ranged between 30 and 60 Gy.
In all cases obliteration of the vessels or at least prevention of further progression could be achieved. In case of bilateral vascularisation both grafts remained clear, the papilloma disappeared one month after treatment. No radiation side effects could be observed until now.
Radiotherapy of vascular diseases of the anterior segment should be limited to those cases in which conservative treatment proved unable to prevent further progression of vascularisation.
The single dose should be 15 Gy and the total dose should not exceed 60 Gy because of the high incidence of radiation induced side effects even when using therapeutic doses.
