Zusammenfassung
Wir berichten über einen Patienten, bei dem nach anfänglich erfolgreich therapierter beidseitiger Chlamydienkonjunktivitis ein Rezidiv ohne nachweisliche Reinfektion auftrat. Im Laufe der Behandlung entwickelte der Patient eine Überempfindlichkeit gegenüber verschiedenen antibiotischen Substanzen und Konservierungsmitteln. Es kam zur Ausbildung eines trophischen Ulkus links, so daß eine perforierende Keratoplastik durchgeführt werden mußte. Die histologische und ultrastrukturelle Untersuchung des Läppchens zeigte ulkustypische morphologische Veränderungen im Randbereich des Läppchens, während im Zentrum ein subepithelialer Pannus aus Fibroblasten, Leukozyten und Stromakollagen auffiel der jedoch weitgehend gefäßfrei war. Im darüberliegenden Epithel zeigten sich C. trachomatis-Elementarkörperchen im Zytoplasma einzelner, intensiv degenerierter Zellen.
Infolge bakterieller Superinfektion entstand am rechten Auge ein Hornhautulkus, das eine perforierende Keratoplastik notwendig machte. Die licht-und elektronenmikroskopische Untersuchung dieses Läppchens ergab das typische Bild eines bakteriell bedingten Ulkus, wobei im tieferen Stroma kokkoide Bakterien zwischen den Kollagenlaminae gefunden wurden.
Summary
We report on a patient who suffered from recurrent chlamydial keratoconjunctivitis in both eyes despite sufficient therapy. In the course of treatment the patient developed hypersensitivity against several antibiotics and preservatives. A trophic ulcer occurred in the left cornea and a perforating keratoplasty had to be carried out. The histologic and ultra-structural examination of the corneal button showed the typical morphologic changes of a corneal ulcer in the periphery, while a subepithelial pannus, consisting of fibroblasts, leucocytes and stromal collagen was found in the center. Unlike the trachomatous pannus it was nearly free of vessels. There were chlamydial elementary bodies in the cytoplasm of single, extremly degenerated cells of the overlaying epithelium.
Bacterial superinfections lead to an ulcer in the right eye and therefore a perforating keratoplasty had to be done. The light- and electron microscopic examination of this corneal button showed the typical picture of a bacterial ulcer, with coccoid bacteria between the collagen-sheets of the deep corneal stroma.
