Zusammenfassung
Die Orbita wird im wesentlichen aus zwei anatomischen Gewebsbereichen vom Tumoreinbruch bedroht. Aus dem Gesichtsschädelinneren wachsen die Karzinome aus der Mund-und Nasennebenhöhlenregion in die Orbita ein. Aus der periorbitalen Hautregion sind es besonders Tumoren und Tumorrezidive im Bereich des medialen Lidwinkels, die mit Knochendestruktion in die Augenhöhle einwachsen. Im Moment der Orbitainfiltration wird die Diagnostik und die operative Therapie zum interdisziplinären Problem, dessen Lösung möglichst in einer Erhaltung des wertvollen Sehorgans Augapfel, mindestens aber in der Entfernung des Tumors im Gesunden liegen muß. Schon in der Diagnostik bewährt sich die Zusammenarbeit zwischen Kieferchirurgie und Ophthalmologie, welche neben den üblichen ophthalmologischen Untersuchungen besonders die neuen bildgebenden Verfahren wie 3 D-Rekonstruktion des Gesichtsschädels oder die Kernspintomographie beisteuern kann. Durch exakte interdisziplinäre Diagnostik wird die Indikation zur Operation gestellt, welche im Idealfall ebenfalls im Sinne eines einzeitigen, gemeinsamen Vorgehens durchgeführt wird.
Summary
Essentially, the threat of tumour invasion into the orbita arises from two anatomic tissue sources. From the internal facial skull carcinomas from the mouth and paranasal sinuses invade the orbita; from the periorbital region of the skin tumours and their recurrences — particularly of in the region of the medial angle of the eye — grow into the orbital cavity during progressive bone destruction. Infiltration of the orbita makes diagnosis and surgery a multidisziplinary problem, with the main objective of preserving the precious visual organ — the bulbus; at least the removal of the tumour from the healthy region should be achieved. From the stage of diagnosis on the cooperation of maxillofacial surgery and ophthalmology has proven valuable, the latter contributing not only the established ophthalmological examinations but, particularly, the new visual methods such as three-dimensional imaging of the facial skull or NMR. Indication of surgery is determined by exact interdisziplinary diagnosis. Whenever, possible, surgery should be performed in one single cooperative step.
