Zusammenfassung
Retrospektiv wurde bei 50 Augen, die aufgrund einer hochbullösen und/oder einer durch posterior gelegene Defekte verursachten Netzhautabhebung mit pneumatischer Retinopexie oder mit einem mit einer radiären Plombe kombinierten Verfahren versorgt wurden, die postoperative Sehschärfe innerhalb der ersten 6 Wochen und nach mehr als 6 Monaten verglichen, wobei Augen mit präoperativ anliegender und abgehobener Makula getrennt beurteilt wurden. Bei präoperativ anliegender Makula erlangten unabhängig von der Operationstechnik 60% der Augen innerhalb der ersten 6 Wochen und 90% nach mehr als 6 Monaten wieder die präoperative Sehschärfe. In Fällen mit präoperativ abgehobener Makula erreichten 86% der mit pneumatischer Retinopexie versorgten Augen eine Sehschärfe von mehr als 6/60 verglichen mit 66% der mit einer permanenten Plombe operierten, nach 6 Monaten jedoch 80% der Augen mit radiärer Plombe gegenüber 71% mit pneumatischer Retinopexie.
Anatomisch wurde eine primäre Heilung in 85% (22/26) mit einer permanenten Plombe und in 71 % (17/24) mit pneumatischer Retinopexie erzielt. In 12.5% (3/24) der letztgenannten Fälle war eine Kataraktoperation zur visuellen Rehabilitation erforderlich. Die schnellere und bessere Erholung des Sehvermögens nach pneumatischer Retinopexie, wie mehrfach postuliert, stellt kein Argument für die Bevorzugung dieses Verfahrens in der Netzhautchirurgie dar.
Summary
Retrospectively we compared the visual acuity of 50 eyes 6 weeks and more than 6 months postoperatively, following either pneumatic retinopexy or a radial scleral buckling procedure. In all cases retinal detachment was either bullous and/or caused by tears located posteriorly to the equator. Eyes with preoperatively attached or detached macular region were followed-up separately. In case of preoperatively attached macula preoperative visual acuity was achieved postoperatively in 60% of the eyes after 6 weeks and 90% after 6 months regardless of the technique used for surgery. Of the eyes with preoperatively detached macula 86% of the pneumatic retinopexy cases achieved at least 20/ 200 after 6 weeks compared with 66% of the eyes with a scleral buckle. After 6 months 80% of the scleral buckle group gained a visual acuity of 20/200 or more, compared with 71 % of the cases with pneumatic retinopexy. Primary anatomic success was achieved in 85% with scleral buckling and 71% with pneumatic retinopexy. In 12.5% of the cases with pneumatic retinopexy cataract surgery has to be performed for full visual rehabilitation. A more rapid and better recovery of visual acuity following pneumatic retinopexy, as it was postulated in the past, is no longer an argument for the prevalence of this technique in retinal detachment surgery.
