Chirurgische Behandlung der okulären Toxocariasis

Zusammenfassung

Bei Endophthalmitis und proliferativer Vitreoretinopathie (PVR) mit Traktions- und/oder rhegmatogener Netzhautablösung kombiniert, die durch Toxocara canis hervorgerufen werden, ist die Pars plana Vitrektomie indiziert. Bei den durch periphere Larvengranulome verursachten Pars planitis-Fällen kann die transsklerale Kryokoagulation in der Verhinderung der Rezidiven und der Beförderung des Absterbens der Larve effektiv sein. Transsklerale Kryokoagulation wurde zweimal, Vitrektomie fünfmal erfolgreich durchgeführt. In allen 7 Fällen wurde eine Sehschärfeverbesserung erzielt und in den durch Netzhautablösung komplizierten Fällen war die Netzhaut wieder angelegt. Trotzdem rezidivierte die Entzündung in 2 Fällen, in einem Fall kam es zur Enukleation.

Summary

Pars plana vitrectomy is indicated in proliferative vitreoretinopathy (PVR) combined with tractional and/or rhegmatogenous retinal ablation and in endophthalmitis, both of them caused by Toxocara canis. In pars planitis elicited by the peripheral granulomas of the larva transscleral cryocoagulations can be effective in the prevention of recurrences and promotion of the dying away of the larvae. Transscleral cryocoagulation and vitrectomy were performed successfully in 2 and 5 cases, respectively. In all of the 7 cases visual improvement was achieved and the retina reattached in the cases combined with retinal detachment. In spite of this, inflammation recurred in 2 cases, and the eye had to be removed in one case.
