Zusammenfassung
Dieser Bericht gibt einen Überblick der Studien mit dem multifokalen Elektroretinogramm (mfERG) bei früher und später altersabhängiger Makulopathie (AMP). Die Experimente umfassten unter anderem die Neuentwicklung eines stäbchenvermittelnden mfERGs sowie die Anwendung neuer computergesteuerter Analysemethodiken. Die bisherigen Ergebnisse zeigen eine signifikante Beeinträchtigung des Stäbchensystems bei früher AMP und lassen vermuten, dass diese zuerst beeinträchtigt sind. Eine Beobachtung, die bis jetzt nur histopathologisch und subjektiv mit psychophysischen Tests, nicht jedoch mittels objektiver Funktionsdiagnostik, gezeigt werden konnte. In Zukunft könnte eine Frühdiagnostik der Stäbchenfunktion hilfreich bei der Verlaufskontrolle der frühen AMP sein. Auch könnte diese Technik eventuell in Therapieansätzen, die speziell auf die Erhaltung der Stäbchenfunktion gerichtet sind, unterstützend sein. Weiters könnte das multifokale Elektroretinogramm auch nützlich in der Erfassung des Funktionszustandes der Makula nach photodynamischer Therapie sein und eventuell in der Wiederbehandlung mitentscheiden.
Conclusion
This paper gives an insight to the studies done with the multifocal electroretinogram in early and late agerelated maculopathy (ARM). The experiments included the development of a rod-mediated mfERG as well as the use of a new computerized analysis method. The results have shown a significant impairment of the rod-mediated mfERG in early ARM. These original findings by using an objective test for measuring the rod-mediated function support histopathological and psychophysical evidence of a preferential rod vulnerability in early ARM. In future the rod-mediated mfERG might be helpful in the early detection of ARM and might indicate new therapies targeting the rods. Cone- and rodmediated mfERGs may be useful in assessing macular function after photodynamic therapy and important in retreatment decisions.
