Zusammenfassung
Hintergrund
Die Überleitung der Medizinischen Universität nach dem neuen UG 2002 führt zu einer neuen Konkurrenzsituation, in der sich die österreichischen Universitäten einem globalen Wettbewerb stellen werden müssen. Ziel dieser Arbeit ist der Vorschlag einer strukturierten Ausbildung auf dem Gebiet der Ophthalmo-Onkologie.
Methode
Nach der ophthalmologischen Grundausbildung (im angloamerikanischen Gebiet dem Residency entsprechend) werden nach einem strukturierten Rasterzeugnis die benötigten Fertigkeiten, eingeteilt in diagnostische (z. B. indirekte Ophthalmoskopie) und therapeutische (Verankern eines Ruthenium-Applikators, selbstständige Durchführung einer Gamma-Knife-Therapie oder einer Thermotherapie) erlernt.
Ergebnis
Mit diesem Curriculum, im Angloamerikanischen einem „Fellowship“ entsprechend, ist ein stufenförmiges Erlernen zunehmend höherer Verantwortlichkeiten möglich. Großer Wert wird auf die Begleitung durch eine(n) erfahrene(n) Kollegen(in) gelegt. Am Ende der Ausbildung steht die eigenverantwortliche Therapieentscheidung sowie die selbstständige und eigenverantwortliche Durchführung von Therapien. Die höchste Stufe ist die Beurteilung und das Management von Therapieversagern (Rezidiven oder Nichtansprechen von Tumoren).
Schlussfolgerung
Ein streng formuliertes Rasterzeugnis ist die Grundvoraussetzung für eine qualitativ hochwertige Ausbildung. Erst durch eine strukturierte Ausbildung auf Basis einer gesamtophthalmologischen Ausbildung werden wir im globalen Wettbewerb bestehen können.
Summary
Background
In January 2004 the Medical University was founded in Graz. According to the University Organisation Law (UG 2002) the new Medical University will be in competition with other universities in Europe and overseas in the future. Theaim of this paper is a suggestion of a curriculum in ophthalmo-oncology in Austria.
Method
After having finished residency the young collegue has to learn diagnostic skills (like indirect ophthalmoscopy and A- and B-scan echography) in his first year of specialisation. In the second year therapeutic skills like suturing of a Ruthenium 106 plaque, Gamma Knife radiosurgery or transpupillary thermotherapy (TTT) should be aimed at.
Results
According to this curriculum corresponding to a fellowship in angloamerican countries the resident can learn skills with an graduated responsibility. During the fellowship a senior resident or experienced associate professor has to guide and supervise the fellow. At the end of the curriculum the fellow should be able to perform therapies by his own. The highest level of education is the management of failures, recurrances and side effects.
Conclusion
In our opinion a curriculum is a basic condition for a high quality education in a specialized field like ophthalmo-oncology. Without a structured education we will not be able to survive in a world wide competition.
