Zusammenfassung
Spezifische IgM-Antikörper sind bereits 4–7 Tage nach Infektion im Serum nachweisbar und persistieren maximal 3 Monate. Die Aussagekraft eines SerumIgM-Tests zur Diagnose von Chlamydieninfektionen bei Erwachsenen ist noch unklar, unumstritten ist dagegen seine Wertigkeit bei Neugeborenen-Pneumonien. Ein neuer Immunoperoxidase-Assay auf chlamydienspezifisches IgM soll auf seine Brauchbarkeit zur Diagnostik von okulogenitalen Chlamydieninfektionen getestet, und die Frage nach der Wertigkeit eines IgM-Nachweises überhaupt, diskutiert werden.
Es wurden Sera von 120 Personen mit positiven chlamydienspezifischem Serum-IgA und- IgG getestet. Es waren dies 87 Patienten mit Konjunktivitis und 11 Patienten mit Iritis, deren Beschwerden maximal 4 Wochen bestanden. Au-ßerdem wurden 22 Mütter von Neugeborenen mit chlamydienbedingter Konjunktivitis untersucht.
Bei allen Patienten mit okulärer Symptomatik wurden Bindehautabstriche auf Chlamydia trachomatis untersucht. Die Antikörperbestimmung im Serum erfolgte mittels Immunoperoxidase-Assays auf IgA und IgG bzw. IgM in Serumverdünnungen von 1:16 (IgM, IgA), 1:32 (IgM), sowie 1:64 und 1:128 (IgG).
Bei 18 Personen wurde ein positiver IgM-Titer gefunden: 12 Patienten mit Konjunktivitis, 1 Patient mit Iritis und 5 Mütter von chlamydieninfizierten Neugeborenen. Bei 8 Patienten mit Konjunktivitis gelang ein Antigennachweis, nur 4 von ihnen wiesen IgM auf. IgM-Antikörper ohne Erregerisolierung wurden bei 8 Patienten mit Konjunktivitis und dem Iritis-Patienten gefunden. Dagegen waren 4 Patienten mit durch Antigennachweis gesicherter Chlamydienkonjunktivitis IgM-negativ. Ein Serum-IgM-Test kann also bei der Diagnose okulärer Chlamydieninfektionen lediglich unterstützenden Charakter haben.
Summary
Specific IgM-antibodies against Chlamydia trachomatis occur in the serum 4–7 days after infection and persist up to 3 months. The diagnostic value of a serum-IgM-test for chlamydial infections of adults is not clear up to now, but it is a very useful diagnostic aid for pneumoniae of newbornes and infants. Therefore a new immunoperoxidase assay should be tested for its value in diagnosis of oculogenital infections.
Sera of 120 persons, which were positive for antichlamydial IgA and IgG were tested: 87 patients with conjunctivitis and 11 patients with iritis, whose ocular symptoms existed no longer than 4 weeks. Moreover 22 mothers of newbornes with chlamydial conjunctivitis were tested.
Conjunctival smears of all patients with ocular symptoms were examined for chlamydial elementary bodies. The evaluation of specific IgM, IgA and IgG was done with immunoperoxidase assays at serum-dilutions of 1:16 (IgM, IgA), 1:32 (IgM), and 1:64 and 1:128 (IgG). A positive IgM-titer was found in 18 of the tested sera: 12 patients with conjunctivitis, 1 patient with iritis, and 5 mothers of chlamydiainfected babies. A chlamydia-positive conjunctival smear was seen in 8 patients with conjunctivitis, but only 4 of them were IgM-positive. In 9 IgM — positive patients — 8 with conjunctivitis and 1 with iritis-no antigen was found. On the other hand 4 patients with positive conjunctival smears showed a negative result in IgM-testing. According to these findings a serum-IgM-test may only be an additional aid for the diagnosis of oculogenital chlamydial infections.
