Samenvatting

76 bij het Apotheek Zorgbureau aangesloten apothekers in de regio Groene Hart hebben onlangs een initiatief gelanceerd om ‘medicatie aan huis’ voor de patiënt veiliger en gemakkelijker te organiseren. De ziekenhuizen en thuiszorginstellingen in de regio (totaal 1,2 miljoen inwoners) hebben zich bij de plannen aangesloten, zodat één centraal loket ontstaat voor zorgverleners en ook patiënten.
