Heupfracturen, van spoed naar goed: multidisciplinaire aanpak en patiëntgericht programma ter verbetering van zorg bij traumatische heupfractuur

Samenvatting

Het Canisius Wilhelmina Ziekenhuis te Nijmegen heeft met zijn partners een nieuw transmuraal kwaliteitsproject ontwikkeld voor de behandeling van de patiënt met een gebroken heup. Multidisciplinaire aanpak en continue verbetering van alle relevante zorgfacetten vormen de pijlers van het Collum Care-project. De kwaliteit van zorg, de logistiek op de afdeling Spoedeisende Hulp (SEH), de verpleegafdeling en het OK-complex zijn verbeterd en de ligduur in het ziekenhuis is gedaald. Wel heeft er verplaatsing van zorg plaatsgevonden van ziekenhuis naar verpleeghuis. Voor alle betrokken disciplines, zowel intra- als extramuraal, is de zorg voor de patiënt met een gebroken heup aantrekkelijker geworden.

Abstract

A new transmural care project has been developed at the Canisius Wilhelmina Hospital in Nijmegen for the treatment of patients with hip fractures. A multidisciplinary approach and a continuous process of monitoring are the base of the Collum Care project. Improvements are achieved in the quality of care, in the logistic process of the emergency unit and the OR unit, whereas the length of stay in the hospital has been reduced. A substitution of hospital care towards nursing home care has taken place. Caring for the patient with a hip fracture has become more attractive for workers in- and outside the hospital.
