Samenvatting

Kinderen met juveniele reuma worden in het Wilhelmina Kinderziekenhuis behandeld door de kinderarts. Maar zodra ze 18 jaar zijn, gaan ze naar de reumatoloog. Om deze overgang makkelijker te maken, worden de kinderen vanaf hun zestiende begeleid door een transitiecoördinator.
