Psoriasis: behandelmogelijkheden in de huisartsenpraktijk

Samenvatting

Patiënten met milde tot matig ernstige psoriasis vulgaris kunnen goed door de huisarts behandeld worden. Uitgebreide voorlichting aan en intensieve begeleiding van de patiënt is daarbij van groot belang. Patiënten met psoriasis hebben vaak psychosociale stress en een (sterk) verminderde kwaliteit van leven. Het is belangrijk dat de huisarts naar dit aspect informeert, het bespreekt en het zo nodig in het behandelplan betrekt.

De medicamenteuze behandeling bestaat in de huisartsenpraktijk uit vitamine-D3-derivaten (calcipotriol, calcitriol), dermatocorticosteroïden of een combinatie daarvan. Een momenteel veelgebruikte en effectieve behandeling bestaat uit een combinatiepreparaat (calcipotriol plus betamethasondipropionaat) gedurende vier weken, gevolgd door monotherapie met calcipotriolcrème of -zalf door de week en het combinatiepreparaat in de weekeinden.

Wanneer enkele regels in acht genomen worden, zullen ernstige bijwerkingen niet optreden.

Door de behandeling kunnen de psoriasislaesies aanzienlijk verbeteren, maar een geheel genormaliseerde huid wordt – ook met de meest effectieve therapie – zelden bereikt. Bovendien is de behandeling altijd symptomatisch en zal zij dus vaak langdurig moeten worden gecontinueerd. In deze bijdrage bespreken we medicamenteuze behandelmogelijkheden van de huisarts voor psoriasis.
