Samenvatting
Hemofagocytaire lymfohistiocytose (HLH) is een zeldzaam ziektebeeld dat onder andere kan ontstaan bij kinderen die zijn gediagnosticeerd met een maligniteit. In dit artikel worden drie casus besproken van patiënten met secundaire HLH bij een maligniteit. Er zijn twee verschillende vormen van HLH. De primaire, genetische, vorm betreft meestal een autosomaal recessieve aandoening. De secundaire vorm kan optreden als reactie op een infectie, auto-immuunziekte zoals reuma, maar ook als reactie op een maligniteit. Bij zowel de genetische als, veelvuldig, bij de verworven vorm is er sprake van een verminderde functie van natural killer-cellen en cytotoxische T-cellen. HLH presenteert zich meestal met aanhoudende koorts en een splenomegalie. Bij patiënten met een HLH ontstaat een hyperinflammatoir proces, dat zonder behandeling vaak een fataal beloop heeft, zowel in de primaire als secundaire vorm. Kenmerkende laboratoriumafwijkingen zijn cytopenie, verhoogd ferritine en/of triglyceride en een verlaagd fibrinogeen. Initieel kan hemofagocytose moeilijk te diagnosticeren zijn. Echter, als er aan voldoende criteria voor de diagnose HLH wordt voldaan, moet direct worden gestart met de behandeling.
Summary
Hemophagocytic lymphohistiocytosis (HLH) is a relatively uncommon disease that can occur in children with a malignancy. In this article we describe three cases of secondary HLH associated with malignancy. There are two distinct forms of HLH. Primary HLH is a genetic form and inherits autosomal recessively. The secondary type of HLH occurs as a reaction to other factors such as infection, autoimmune disease or malignancy. In the primary form and in most of the secondary forms there is an impaired function of natural killer cells and cytotoxic T cells. HLH is characterized by prolonged fever and splenomegaly. However, patients with HLH cannot control the hyperinflammatory response, which can be life-threatening. Relevant biochemical markers related to the disease include pancytopenia, hyperferritinemia, hypertriglyceridemia and/or hypofibrinogenemia. Initially, hemophagocytosis itself can be difficult to identify. If the diagnostic criteria of HLH are met, it is important to start treatment in time.
