Samenvatting
Het polycysteus ovariumsyndroom (PCOS) is een van de meest voorkomende endocriene stoornissen bij premenopauzale vrouwen en meisjes. Hyperandrogenisme, chronische anovulatie en polycysteuze ovaria op echografie zijn de diagnostische kenmerken die in meer of mindere mate aanwezig zijn. Hierbij wordt vaak obesitas, insulineresistentie en het metabool syndroom gezien, hetgeen de noodzaak voor vroege diagnostiek en follow-up bijzonder belangrijk maakt. Op de kinderleeftijd kan een dergelijk beeld herkend worden door een vaak al pre-existent hoog gewicht en een toename in de puberteit, premature pubarche en een vroege menarche waarbij de cyclus na enkele jaren irregulair wordt en de menstruatie uitblijft.We beschrijven een patiënt met enkele moeilijke differentiaaldiagnostische overwegingen voordat de diagnose PCOS wordt gesteld. Naar aanleiding van deze casus wordt ingegaan op de diagnostiek en behandeling van PCOS.
Summary
Polycystic ovary syndrome (PCOS) is one of the most common endocrinopathies in girls and premenopausal women. Hyperandrogenism, chronic anovulation and polycystic ovaries on ultrasound are the diagnostic features present to various degrees. Additionally obesity, insulin-resistance and metabolic syndrome are often present, which stresses the importance of early diagnosis and long follow-up. In childhood, a pre-PCOS is often seen: a preexistent high weight, increasing in puberty, premature pubarche and early menarche followed by a progressively irregular cycle after a few years and later cessation of menstruation.We describe a patient with some difficult differential-diagnostic considerations before the diagnosis PCOS was made. With reference to this case we discuss the diagnostic features and treatment options in PCOS.
