Summary
Home mechanical ventilation may be a therapeutic option in children with chronic respiratory failure. Ideally, this therapy should not be initiated at the time of an acute respiratory failure but electively and after careful considerations have been made whether it is an appropriate therapeutic strategy for a particular patient. Once patient, parents and medical team have decided to pursue home mechanical ventilation, systematic screening and follow-up of the patient are necessary in order to determine individually the appropriate time to start up the support. This article summarises practical guidelines for screening and follow-up of patients with (1) neuromuscular disorders, (2) central hypoventilation, (3) obstructive sleep-disordered breathing or (4) cystic fibrosis.
