Thuisbeademing voor kinderen; de indicatiestelling

Samenvatting

Het aantal kinderen in Nederland dat voor thuisbeademing in aanmerking komt, is in de afgelopen decennia gestaag toegenomen. De beademingszorg voor deze groep van hoogcomplexe kinderen ligt in Nederland in handen van vier Centra voor Thuisbeademing (CTB’s). Om deze zorg uniform te laten plaatsvinden, zijn recent door deze CTB’s richtlijnen opgesteld rond de indicatiestelling en organisatiestructuur voor thuisbeademing. Kennis van de indicatie tot thuisbeademing is belangrijk voor al diegenen die te maken hebben met kinderen die chronisch respiratoir insufficiënt kunnen worden. In dit artikel beschrijven we de indicatiestelling voor thuisbeademing bij kinderen. Deze indicatie wordt vooral gesteld op basis van kliniek en anamnese en/of mate van hypercapnie. Het kind moet daarbij in principe bekend zijn met een onderliggende ziekte, die aanleiding kan geven tot chronische respiratoire insufficiëntie. Ten slotte moet chronische beademing gewenst en zinvol zijn, adequaat en veilig uitvoerbaar en technisch haalbaar zijn.

In the past decades the number of children in the Netherlands that benefit from home mechanical ventilation has increased steadily. The home ventilatory care for this group of children is highly complex and is provided by the four Home Mechanical Ventilation Centres (HMVC’s). In order to deliver uniform care for these patients the HMVC’s have recently reached consensus on guidelines defining the indications and organisation of home mechanical ventilation for children in the Netherlands. Awareness of indication and organisation criteria of home mechanical ventilation is essential for all those who are involved in the treatment of children with a disease that may lead to chronic respiratory insufficiency, to optimize the care for these children. In this article we describe the guideline on indication for children who are eligible for home mechanical ventilation. The indication for home mechanical ventilation in children is established on clinical presentation and medical history together with the existence of (nocturnal) hypercapnia. Finally, home mechanical ventilation must be desirable for child and parents/caretakers, but also clinically effective, safe and technical feasible.
