Samenvatting
Momenteel wordt 24-uurs intraoesofageale pH-meting beschouwd als de gouden standaard om pathologische gastrooesofageale reflux aan te tonen. In de praktijk blijkt de beoordeling van een pH-meting geen gemakkelijke opgave. In dit artikel bespreken we hoe een pH-meting beoordeeld moet worden. We gaan daarbij in op de uitvoering van een pH-meting, de technische problemen die zich tijdens een pH-meting kunnen voordoen, de interpretatie van refluxparameters en het effect van voeding bij zuigelingen met gastro-oesofageale reflux. Tevens bespreken we aan de hand van een aantal voorbeelden de problemen die zich bij de beoordeling kunnen voordoen.
Summary
Oesophageal pH-monitoring is at present considered to be the gold standard to diagnose pathological gastro-oesophageal reflux. In daily practice the interpretation of a pH tracing is not easy. In this article, we discuss the methodology of 24-hour intraesophageal pH-monitoring, the technical problems that can occur, the interpretation of the reflux parameters and the effect of feeding in infants with gastro-oesophageal reflux. With various examples the pitfalls in the assessment of pH-monitoring are shown.
