Samenvatting

In deze H&W treft u een LTA (landelijke transmurale afspraak) aan. Dat is niet de gewoonte, maar voor de nieren maken we een uitzondering. Reden is dat aan deze LTA geen NHG-Standaard ten grondslag ligt. De LTA draagt daarom deels ook het karakter van een standaard: uitgebreide motivatie voor de gemaakte keuzes en een uitgebreid notenapparaat.
