Samenvatting

Huisartsen hebben het vaak over continuïteit in de zorg, maar wat ze dan precies bedoelen is minder vaak duidelijk. Meestal lopen allerlei facetten van continuïteit door elkaar: persoonlijke continuïteit (de relatie van de patiënt met de eigen huisarts), teamcontinuïteit (zorg geleverd door een team zoals een huisartsenpraktijk of een doktersdienst) en continuïteit van informatie. Ik heb het in deze bijdrage over dit laatste aspect. Continuïteit van informatie betekent dat informatie gestructureerd wordt vastgelegd in het medisch dossier en beschikbaar is voor iedere professional die te maken heeft met de zorg voor de patiënt. Bovendien behelst het de toegang tot en de communicatie over deze informatie.1
