Inleiding Er is geen gepubliceerd bewijs dat klachten en symptomen bruikbaar zijn bij het onderscheid tussen een bacteriële en een andere (acute) conjunctivitis. Het doel van dit onderzoek was een diagnostisch model te ontwikkelen dat optimaal voorspelt of een infectieuze conjunctivitis door een bacterie wordt veroorzaakt.
Methoden We onderzochten een cohort van volwassen patiënten zonder contactlenzen met klachten van een rood oog, en (muco)purulente afscheiding en/of dichtgeplakte oogleden. De indextestresultaten (klachten en symptomen) en de referentiestandaard (kweek van de conjunctiva) werden onafhankelijk van elkaar verkregen. De belangrijkste uitkomstmaten waren de voorspelde kans op een positieve kweek, gegeven verschillende combinaties van indextestresultaten, en de oppervlakte (area under the curve, AUC) onder de receiver operating characteristic-curve.
Resultaten Er werden 177 patiënten ingesloten. Uit de logistische regressieanalyse bleek dat de combinatie van dichtgeplakte ogen, jeuk en een voorgeschiedenis van infectieuze conjunctivitis de optimale diagnostische voorspeller was: AUC 0,74 (95%-BI 0,63-0,80). De prevalentie van een positieve kweek van 32% kon met behulp van het model verlaagd worden naar 4% of verhoogd worden naar 77%, afhankelijk van het patroon van indextestresultaten.
Conclusie Met behulp van drie eenvoudige vragen kan de waarschijnlijkheid van een bacteriële oorzaak van infectieuze conjunctivitis worden verhoogd of verlaagd. Het resultaat van dit onderzoek kan ertoe leiden dat huisartsen meer gericht antibiotica voorschrijven.
