Samenvatting
De remodeling-theorie, zoals verwoord door Van den Brink en Bootsma in H&W 2003(1):33-36 biedt de huisarts weinig praktische aanknopingspunten voor behandeling van astmatici, die met de ‘gewone aanpak’ (stap 1 en 2 van de NHG-richtlijnen) niet goed onder controle te krijgen zijn. De schrijvers stellen dat remodeling met name door chroniciteit bepaald wordt, maar ook op jonge leeftijd kan vóórkomen. Remodeling zou bijdragen aan persisterende obstructie en aan de symptomen van astma, maar ook asymptomatisch of bij lichte vormen van astma kunnen optreden. Er is geen klinisch bruikbare methode om remodeling vast te stellen.
