Omdat obesitas een risicofactor is voor talrijke somatische aandoeningen, zijn preventie en behandeling van overgewicht noodzakelijk. De meest gehanteerde maatregel is lijnen, maar helaas haalt dit op de lange termijn meestal weinig uit. In dit artikel wordt een pleidooi gehouden voor een beperkte doelstelling. Aanlegfactoren beperken de mogelijkheden voor gewichtsverlies. Slechts zelden kunnen blijvend betere resultaten dan ongeveer 10% gewichtsreductie worden geboekt. Op grond van irreële verwachtingen stellen veel obese patiënten zich irreële doelen en eisen. Omdat zij zich niet aan de rigide gedrags- en dieetregels kunnen houden, krijgen zij een gevoel van mislukking, wat weer aanleiding geeft voor eetgedrag.
Een effectieve behandeling heeft als doel het lichamelijke activiteitenniveau te verhogen, een caloriearm dieet met weinig vetten en veel koolhydraten te introduceren en het eetgedrag daadwerkelijk te veranderen. Hierbij kan cognitief-gedragstherapeutische behandeling een belangrijke rol spelen, gericht op uitlokkende prikkels, zelfcontrole, dieetregels, gedragsregels en eventueel onderliggende problematiek. Bij zeer ernstige of therapie-resistente obesitas kunnen chirurgische maatregelen overwogen worden.
