Abnormaal vaginaal bloedverlies

Overvloedig, tussentijds of onregelmatig vaginaal bloedverlies berust vaak op een betrekkelijk onschuldige hormonale ontregeling of is een bijwerking van anticonceptie, antistolling of andere medicatie. Met behulp van anamnese, inspectie van de vulva, speculumonderzoek en vaginaal toucher schat men de kans in op organische oorzaken zoals zwangerschapscomplicaties, pelvic inflammatory disease, vaginitis, cervicitis, poliepen en myomen. Bij postmenopauzaal bloedverlies zijn maligniteiten niet zeldzaam (circa 8%). Minstens drie maanden overvloedig vloeien is reden voor bloedonderzoek op ferriprieve anemie. Een zwangerschapstest is vaak noodzakelijk om vloeien als complicatie van een niet-bekende zwangerschap uit te sluiten. Bij contactbloedingen, tussentijds of onregelmatig bloedverlies en postmenopauzaal bloedverlies is een cervixuitstrijk aangewezen om een (pre) maligne cervixafwijking uit te sluiten. Stollingsscreening wordt aanbevolen wanneer de anamnese op een familiaire stollingsstoornis of bloedingsneiging wijst en bij menorragie sinds de menarche. Indien soa anamnestisch niet kunnen worden uitgesloten is een chlamydiatest geïndiceerd bij tussentijds of onregelmatig bloedverlies. Een vaginale echoscopie is geïndiceerd bij elk postmenopauzaal bloedverlies ter uitsluiting van een endometriumcarcinoom, in de fertiele fase bij vergrote adnexen of ter opsporing van intracavitaire afwijkingen. Nader specialistisch onderzoek bestaat uit aspiratiebiopsie of hysteroscopie met biopsieën en histologisch onderzoek. Diagnostische curettage is hierdoor overbodig geworden.
