Samenvatting
In 1993 stelde de Inspectie van de Volksgezondheid dat aan vrouwen met zwangerschapswens geadviseerd moet worden foliumzuurtabletjes te gebruiken van vier weken voor de conceptie tot acht weken erna. Dit leidt tot een vermindering van het risico op foetale neurale-buisdefecten met 50-70%. In 1995 volgde een informatiecampagne onder huisartsen en andere professionals en publiek. Sindsdien nam de frequentie van foliumzuurgebruik rond de conceptie in Nederland tot 1998 toe, waarna het stabiliseerde. Sinds 1998 gebruikte ruim 60% van de zwangeren foliumzuur in (een deel van) de geadviseerde periode, en 36% gebruikte het gedurende de gehele geadviseerde periode. Gegevens uit Nederlandse registraties geven aanwijzingen dat de frequentie van neurale-buisdefecten inderdaad gedaald is. Inmiddels lijkt foliumzuur ook te beschermen tegen andere aangeboren afwijkingen, zoals aangeboren hartafwijkingen, gespleten lip en verhemelte, reductiedefecten van de ledematen en afwijkingen van de urinewegen.
In het buitenland is verrijking van voedsel met foliumzuur sinds het begin van jaren negentig steeds meer toegepast, in Nederland is dat verboden. Een recent rapport van de Nederlandse Gezondheidsraad gaat niet verder dan de aanbeveling verrijking toe te staan van producten die gericht zijn op de doelgroep. De minister van VWS adviseerde in 2001 dan ook voorlopig door te gaan met profylaxe met tabletten.
