Samenvatting

Doel Evaluatie van ‘transmurale spirometrie’ als diagnostische faciliteit voor huisartsen in de regio Alkmaar.

Methode Alle 410 aanvragen van huisartsen voor spirometrieonderzoek in een periode van één jaar werden retrospectief beoordeeld op indicatie, uitslag, beoordeling door de longarts en ingesteld beleid naar aanleiding van de uitslag.

Resultaten De helft van de huisartsen maakte gebruik van transmurale spirometrie. Daarbij waren de toegepaste indicaties ruimer dan de NHG-standaarden aangeven. Bij de helft van de aanvragen was er een afwijkende, overwegend obstructieve longfunctiewaarde. De beoordeling van de uitslag door de longarts verschilde soms van de interpretatie volgens de NHG-standaard. Het beleid dat was ingesteld naar aanleiding van de uitslag was niet altijd conform de standaard.

Conclusie De indicatiestelling voor transmurale spirometrie is niet altijd conform de NHG-standaard astma/COPD. Transmurale afspraken over interpretatie van de spirometrieuitslag en het in te stellen vervolgbeleid lijken gewenst.

Samenvatting

Goal Evaluation of an ‘open access’ spirometry facility in a general hospital, with particular emphasis on whether the guidelines of the Dutch College of General Practitioners are being followed.

Methods Retrospective design. All 410 requests for spirometry by general practitioners between September 1998 – October 1999 were evaluated. An analysis was made of indications for the test, results, assessment of the results by a lung specialist and management strategy.

Results In the first year the open access facility was used by half the GPs in the region. A broader spectrum of medical grounds for spirometry was used than could be expected according to the guidelines. In half the patients a predominantly obstructive lung function was found. The assessment of the test result by the lung specialist is not always in line with the official GP guidelines, as was the case with the management plan after spirometry.

Conclusion Open access spirometry is a useful diagnostic tool for the general practitioner although improvement of the medical grounds for spirometry is needed, as is adherence to national guidelines.
