Samenvatting
De floating shoulder is gedefinieerd als een ipsilaterale fractuur van de clavicula en de scapulahals en wordt gezien als een instabiel letsel waarbij de beide fracturen kunnen disloceren. Een floating shoulder wordt vrijwel altijd veroorzaakt door een hoogenergetisch trauma en kan, indien gemist in een acute situatie, pijnlijke gevolgen hebben. Het kan leiden tot chronische pijn, functiebeperking en krachtsverlies. Er is nog geen (eenduidige) consensus over de behandeling van de floating shoulder. Een aantal kleine studies pleit in geval van een niet-gedisloceerde fractuur voor conservatieve behandeling. Aan de andere kant zijn er ook studies die pleiten voor operatief ingrijpen. De behandeling is afhankelijk van de bijkomende verwondingen en van de mate van dislocatie van de fractuur.
Onze casus betreft een jongeman van 19 jaar met drie onderbrekingen van het superior shoulder suspensory complex (SSSC): een fractuur van de scapulahals, een fractuur van het acromion en een diastase van het AC-gewricht. Er werd besloten deze potentieel instabiele, maar niet-gedisloceerde, situatie te stabiliseren door fixatie van de acromionfractuur met osteosynthesemateriaal.
Abstract
The defi nition of a fl oating shoulder is a double interruption of the shoulder girdle and it is considered as an unstable injury where both fractures can get displaced. The cause of a fl oating shoulder is almost always a high energy trauma. If the injury is not properly diagnosed in the acute setting, it can have very painful results, such as chronic pain, functional impairment and reduction of muscle strength. There is no general consensus about the treatment of a fl oating shoulder. A number of small studies plead for a conservative treatment if there is no dislocation of the fracture. On the other hand there are a couple of studies which favour surgical treatment. The treatment of choice depends on the severeness of the associated injuries and possible displacement of the glenoid.
Our case involves a 19-year-old man who had a triple disruption of his shoulder girdle: a fracture of the scapular neck, a fracture of the acromion and a disruption of the AC joint. In this case it was decided to treat this potentially unstable situation with fi xation of the acromion fracture using internal fi xation.
