Samenvatting
Letsel van de abdominale aorta na stomp buiktrauma is zeldzaam. Snelle herkenning, diagnostiek en behandeling kunnen de uitkomst gunstig beïnvloeden. Aan de hand van een casus worden de etiologie, kliniek, aanvullende diagnostiek en behandeling beschreven.
De klinische trias van verminderde tot afwezige pulsaties aan de benen, neurologische uitval aan de benen en een acute buik vereist nader onderzoek naar de abdominale aorta. Aanvullende diagnostiek bestaat allereerst uit een duplex-echo van de beenarteriën. Afhankelijk van de hemodynamische toestand van de patiënt kan een echo van het abdomen, een CT van het abdomen en/of CT-angiografie van het abdomen letsel uitsluiten dan wel aantonen en informatie bieden voor de te kiezen behandelstrategie. Deze bestaat in geval van geïsoleerd aortaletsel zonder indicatie voor laparotomie uit een endovasculaire benadering. In geval van uitgebreider vaatletsel en/of indicatie voor laparotomie wordt een open behandeling met interpositie van een prothese geadviseerd. Bij intra-abdominale contaminatie door bijvoorbeeld een perforatie van een hol orgaan, verdient een endarteriëctomie met primaire naad, veneuze patch of veneus interponaat de voorkeur.
De morbiditeit bij letsel van de abdominale aorta wordt met name bepaald door de neurologische restschade. Als deze gebaseerd is op een acute perifere ischemie, kan door een reperfusie binnen zes uur de schade beperkt blijven.
Abstract
Traumatic injury of the abdominal aorta is uncommon. Early recognition, diagnosis and treatment can positively effect outcome. Etiology, clinical symptoms, diagnosis and treatment will be discussed in reference of a case report.
The clinical triad of decreased or absent pulsations in the lower extremities, neurological deficit and abdominal pain prompts further investigation of the abdominal aorta. Diagnostic tools are Doppler ultrasound of the lower extremities. A CT/ CT-angiogram scan of the abdominal aorta can provide the diagnosis and is a tool in determining the treatment strategy. Treatment consists of endovascular approach in the case of isolated abdominal aorta injury without an indication for a laparotomy. In case of more extensive injury to the aorta or other intra-abdominal organs warranting a laparotomy a synthetic conduit is indicated. If there is intra-abdominal contamination of the abdomen due to for example a perforation of a viscous organ, endarterectomy with primary closure, venous patching or venous interponate is indicated.
Morbidity due to injury of the abdominal aorta is mainly associated with neurological deficit. If caused by acute peripheral ischemia it can be reduced by early recognition and reperfusion within 6 hours.
