Verslavingsproblematiek in een forensisch–psychiatrisch instituut bezien en bewerkt vanuit groepsdynamisch perspectief

Abstract

Het aantal drugsverslaafden onder patiënten die ter beschikking zijn gesteld neemt toe. Wanneer de patiënten ook tijdens hun opname in een TBS–kliniek middelen blijven gebruiken, wordt een effectieve behandeling onmogelijk. In dit artikel beschrijven we de wijze waarop in de Dr. Henri van der Hoeven Kliniek met de drugsproblematiek werd omgegaan.

Bions visie op groepsprocessen werd gebruikt om processen te analyseren in de staf– en patiëntengroep, die het drugsprobleem in stand hielden. Getracht werd de sterk aanwezige ‘basic assumption’–activiteit door middel van interventies op verschillende niveaus (de staf, de patiëntengemeenschap, de leefgroepen, een kleine therapiegroep voor drugsverslaafde patiënten) om te buigen in taakgerichte activiteit. Om dit doel te bereiken werd op gestructureerde, realiteitsgeoriënteerde wijze geïntervenieerd, waarbij gebruik werd gemaakt van psycho–educatieve en gedragstherapeutische principes. Doordat er op verschillende niveaus interventies plaatsvonden, versterkten de afzonderlijke interventies elkaars effect.

Summary

Among the patients committed to forensic psychiatric clinics in the Netherlands, the number of patients who are addicted to drugs is on the increase. If these patients continue to take drugs, effective treatment is not possible. In this article the way is described in which the problem of drug abuse by patients was tackled in the Dr. Henri van der Hoeven Kliniek.

Bion's work on group dynamics was used to analyse the group processes that played a part in the problem of drug abuse in the clinic. An attempt was made to change the strong ‘basic assumption’ activity into a task oriented activity by means of therapeutic interventions aimed at several different levels (the staff, the whole patient community, the wards, a small therapy group for drug addicts). In order to achieve this objective, a reality oriented, structured approach was adopted, based on principles derived from psycho–education and behavioural therapy. Since the therapeutic interventions took place within different but overlapping groups in the clinic, the individual interventions had a mutually enhancing effect.
