Primair pompen bij peuters: start insulinepomptherapie bij diagnose

Samenvatting

Diabetes mellitus type 1 is een groeiend probleem binnen de kindergeneeskunde. Jonge kinderen zijn vaak moeilijk in te stellen en hebben een relatief grote kans op restschade bij hypoglykemieën. Diverse studies hebben aangetoond dat een intensieve behandeling met insuline niet alleen een beter HbA1c bewerkstelligt, maar ook tot significant minder complicaties op de lange termijn leidt. Dit wordt tegenwoordig geduid als ‘het metabole geheugen’. Primair intensieve therapie door middel van een insulinepomp bij nieuw gediagnosticeerde patiënten onder de 5 jaar met diabetes verdient derhalve wellicht de voorkeur. In deze studie worden de resultaten van negen kinderen met primaire insulinepomptherapie vergeleken met kinderen die de standaardtherapie kregen in onze kliniek. Hieruit blijkt dat in de groep met insulinepomptherapie niet alleen het HbA1c lager was, maar dat er eveneens sprake was van een evident lagere insulinebehoefte. In het licht van het metabole geheugen lijkt er bij jonge kinderen derhalve veel winst te behalen en pleiten wij voor primaire insulinepomptherapie bij nieuwe diabetespatiënten onder de 5 jaar.

Summary

Type 1 diabetes mellitus is an increasing problem in pediatric care. In infants and young children with diabetes glucose levels are often difficult to manage and the children are at increased risk of complications as a consequence of frequent hypoglycemias. In the past, several studies have shown that intensive insulin treatment results in a lower HbA1c and also significantly diminishes long-term complications. Currently, this principle is referred to as ‘the metabolic memory’. Direct intensive treatment by means of insulin pump therapy might be the treatment of first choice in newly diagnosed pediatric diabetes patients. In this study we compared the results of nine children with insulin pump therapy with children receiving standard insulin treatment in our hospital. It shows that the HbA1c is lower in the insulin pump group and moreover, the total insulin intake was also clearly lower compared to the standard therapy group. This study supports our view that young children with newly diagnosed diabetes should directly be put on insulin pump therapy. The principle of ‘the metabolic memory’ further reinforces this.
