Samenvatting

Chronisch hartfalen (CHF) is een gezondheidsprobleem met een hoge prevalentie en incidentie onder ouderen en hangt samen met herhaalde ziekenhuisopnames. 2 Het gebrek aan sociale ondersteuning en emotioneel disfunctioneren lijken zowel de symptomen van CHF als ook het zoeken van adequate hulp te beïnvloeden. Het effect van evidence-based thuiszorgprogramma voor patiënten met CHF wordt bedreigd door factoren zoals een slechte therapietrouw (geneesmiddelen en dieet) en een slechte toegang tot (eerstelijns) medische zorg.
