Samenvatting
Geneesmiddelenbewaking, dat wil zeggen het opsporen van ongewenste effecten van geneesmiddelen nadat deze op de markt zijn gebracht, is noodzakelijk omdat het risicoprofiel van een geneesmiddel op het ogenblik van commercialisering nog onvoldoende bekend is. Klinisch onderzoek in het kader van de registratie van een geneesmiddel heeft een aantal beperkingen als het gaat om het detecteren van ongewenste effecten. De aantallen patiënten zijn onvoldoende om zeldzame ongewenste effecten te detecteren, en de inclusiecriteria zijn zo strikt dat de patiënten met de grootste kans op ongewenste effecten – zoals hoogbejaarden en patiënten met onderliggend lijden – meestal uitgesloten worden.
Er zijn verschillende methoden om ongewenste effecten te detecteren als een geneesmiddel eenmaal op de markt is. Gerandomiseerde onderzoeken zijn slechts zelden geschikt. Meestal gaat het om ‘case-reports’ en ‘case-series’, om spontane meldingen van ongewenste effecten aan een geneesmiddelenbewakingscentrum of om niet-gerandomiseerde (dat wil zeggen observationele) onderzoeken. Daarbij moet rekening gehouden worden met veel onzekerheden, onder andere in verband met causaliteit en incidentie, maar vaak moeten beleidsbeslissingen genomen worden op het ogenblik dat er nog veel onzekerheden zijn. Gezondheidswerkers en patiënten dienen op de hoogte te worden gehouden van de ongewenste effecten en zich bewust te zijn van de onzekerheden daaromtrent.
