Kosteneffectiviteit en kostenutiliteit van rugscholen in de bedrijfsgezondheidszorg

Abstract

Doel: De opzet beschrijven van een economische evaluatie naast een gerandomiseerde interventiestudie van een laag-intensieve en hoog-intensieve rugschool in vergelijking met gebruikelijke zorg door de bedrijfsarts.

Opzet: Gerandomiseerde interventiestudie en economische evaluatie.

Onderzoekspopulatie: Werknemers die tussen de drie en zes weken verzuimen vanwege niet-specifieke lage-rugpijn.

Behandelingen: Een hoog-intensieve rugschool: een trainingsprogramma gebaseerd op cognitief-gedragsmatige principes. Dit programma werd twee keer per week, acht weken lang uitgevoerd. Een laag-intensieve rugschool: deze is vergelijkbaar met de oorspronkelijke ‘Zweedse rugschool’ en werd één keer per week gegeven, vier weken achter elkaar. Gebruikelijke zorg: die bestond uit begeleiding van de zieke werknemer door de bedrijfsarts volgens de richtlijn ‘Handelen van de bedrijfsarts bij werknemers met lage-rugklachten’.

Effectmaten: Gedurende twaalf maanden werd er informatie verzameld over de primaire effectmaat ziekteverzuim en de secundaire effectmaten: pijnintensiteit, functioneren, bewegingsangst, ervaren herstel en de waarde die men hecht aan de eigen gezondheidstoestand (utiliteit) en alle directe en indirecte kosten.

Discussie: Resultaten van dit onderzoek kunnen hulpverleners en beleidsmakers ondersteunen bij het maken van de juiste keuzes voor de behandeling van rugklachten.
