Samenvatting
Studiedesign: Meta-analyse.
Doelstelling: De diagnostische nauwkeurigheid van de klinische tests voor een voorste-kruisband (VKB-)ruptuur vaststellen.
Achtergrond: De VKB is een van de meest aangedane structuren van de knie. Aangezien de prevalentie hiervan toeneemt, is er behoefte aan adequate klinische besluitvorming. In deze meta-analyse is de wetenschappelijke literatuur onderzocht om de diagnostische nauwkeurigheid van het klinische onderzoek voor het vaststellen van een VKB-ruptuur te analyseren.
Methoden: Er werden zoekstrategieën in PubMed (1966-april 2005), EMBASE (1989-april 2005) en CINAHL (1982-april 2005) uitgevoerd. Voor data-analyse werden studies geselecteerd die de nauwkeurigheid van ten minste één klinische diagnostische test beschreven en vergeleken met het klinisch onderzoek van de knie met een gouden standaard, zoals artroscopie, artrotomie of MRI. Het zoeken werd beperkt tot Engelse, Duitse en Nederlandse literatuur.
Resultaten: Achtentwintig studies die de nauwkeurigheid van de klinische tests voor een VKB-ruptuur onderzochten voldeden aan de inclusiecriteria. De studieresultaten waren zeer heterogeen. De lachmantest, met een gepoolde sensitiviteit van 85% (95% CI, 83-87) en een gepoolde specificiteit van 94% (95% CI, 92-95), is de meest valide test om een VKB-ruptuur vast te stellen. De pivot shift is erg specifiek, namelijk 98% (95% CI, 96-99), maar heeft echter een slechte sensitiviteit van 24% (95% CI, 21-27). De schuiflade naar anterieur laat een goede sensitiviteit en specificiteit zien in de chronische fase: respectievelijk 92% (95% CI, 88-95) en 91% (95% CI, 87-94), maar niet in de acute fase: een sensitiviteit van 49% (95% CI, 43-55) en een specificiteit van 58 (95% CI, 39-76).
Conclusie: In het geval van verdenking van een VKB-ruptuur is het aan te bevelen om de lachmantest uit te voeren. Aangezien de pivot shift erg specifiek is in zowel de acute als de chronische fase, is het aan te bevelen om ook deze test toe te passen.
