Diagnostiek en indicatiestelling bij zogenaamde vroege stoornissen

Abstract

In dit artikel worden vroege stoornissen geanalyseerd vanuit een structureel psychoanalytisch gezichtspunt. Hierbij wordt een onderscheid gemaakt in drie gezichtspunten: de drift, het ego en het zelf. Deze drie gezichtspunten worden toegelicht en aan de hand hiervan wordt uiteengezet welk type indicatiestelling voor behandeling volgt uit de diagnostiek van de patiënt en zijn klachten. Het voorstel wordt gedaan om het concept vroege stoornis af te schaffen en dit te vervangen voor een analyse met behulp van deze gezichtspunten. De indicatiestelling voor behandeling wordt hiermee gepreciseerd.

Assessment and indications for treatment in case of so–called early disorders

Starting from a structural psychoanalytic perspective, this article covers the analysis of early disorders. In this analysis a differentiation is made between three viewpoints: the drive, the ego and the self. These three viewpoints are explained and, with the help of these differentiations, indications for the type of treatment are formulated based upon the assessment of the patient and his/her complaints. It is suggested that the concept of early disorders be abandoned, and should be replaced by the analysis of the disorder based upon the viewpoints. This results in the indication for treatment becoming more specific.

Dr. J.J.L. Derksen is klinisch psycholoog en psychoanalytisch psychotherapeut. Hij doceert psychodiagnostiek en psychoanalyse aan de vakgroep Klinische psychologie en persoonlijkheidsleer van de Katholieke Universiteit van Nijmegen en is werkzaam in de eerstelijnsgezondheidszorg. De laatste tien jaar doet hij onderzoek naar en publiceert over persoonlijkheidsstoornissen. Hij is lid en supervisor van de Nederlandse Vereniging voor Psychoanalytische Psychotherapie.
