Fataal astma bij kinderen, risicofactoren en preventie

Summary

A six-year-old girl with a history of asthma, died because of acute bronchoconstriction. Risk factors for fatal asthma are previous respiratory insufficiency, severe asthma, poor perception of dyspnea, misuse of β2-agonists without sufficient anti-inflammatory therapy during exacerbation, undertreatment, poor insight in disease and patient delay. Adequate therapy, recognition of risk factors, regular check-up and clear and written instruction to parents how to handle in case of dyspnea of their child is important in the prevention of fatal asthma.

Samenvatting

Een 6-jarig meisje, bekend met astma, overleed op de spoedeisende hulp ten gevolge van een acute bronchusobstructie. In dit artikel worden risicofactoren besproken voor fataal astma. Met name eerdere respiratoire insufficiëntie, ernstig astma, slechte gewaarwording van benauwdheid, frequent β2-mimeticagebruik zonder voldoende anti-inflammatoire therapie tijdens exacerbatie, onderbehandeling, onvoldoende ziekte-inzicht en het laat zoeken van hulp door de patiënt kunnen een rol spelen. Adequate therapie, het herkennen van risicopatiënten, goede afspraken tussen kinder- en huisarts over controles en duidelijke, ook schriftelijke instructie aan ouders hoe te handelen en wanneer contact op te nemen met een arts bij benauwdheid van hun kind, zijn essentieel in de preventie van fataal astma.
