Summary
Over a 2½-yr period, indication, findings, and complications of 54 paediatric bronchoscopies carried out in our hospital were recorded prospectively. Four patients underwent rigid bronchoscopy for removal of an aspirated foreign body. In the other 50 patients, flexibele bronchoscopy was performed under general anaesthesia while the patient was breathing spontaneously through a laryngeal mask airway. Mean age of the patients (range) was 60 months (5 days-14 yrs). The most common indications for bronchoscopy were severe/therapy-resistant asthma/wheeze (31%), recurrent pneumonia (22%) and progressive stridor (11%). Thirty seven out of 54 bronchoscopies (69%) yielded results with clear diagnostic or therapeutic consequences. The most common findings were stenosis or malacia of the lower airways, which was found in 47% and 50% of patients with therapy-resistant asthma and recurrent pneumonia, respectively. Complications were rare and mild; there were no severe complications. Bronchoscopy is a useful diagnostic procedure in children with severe or therapy-resistant asthma or recurrent pneumonia, and it can be carried out safely in a general hospital by an experienced bronchoscopist and while the patient is under general anaesthesia with monitoring of vital functions.
Samenvatting
In een periode van 2½ jaar werd van 54 bronchoscopieën bij kinderen, die in ons ziekenhuis werden verricht, prospectief indicatie, bevindingen en complicaties vastgelegd. Bij vier patiënten werd starre bronchoscopie uitgevoerd voor verwijdering van een geaspireerd corpus alienum. Bij de overige 50 patiënten werd flexibele bronchoscopie uitgevoerd onder algehele anesthesie en tijdens spontane ademhaling via een larynxmasker. De gemiddelde leeftijd van de patiënten (bereik) was 60 maanden (5 dagen-14 jaar). De meest voorkomende indicaties voor bronchoscopie waren ernstig/therapieresistent astma/piepen (31%), recidiverende pneumonieën (22%) en progressieve stridor (11%). Bij 37 van de 54 bronchoscopieën (69%) werden bevindingen gedaan met duidelijke diagnostische of therapeutische consequenties. De meest voorkomende bevindingen waren stenose of malacie van de lagere luchtwegen, die werd gezien bij 47% van de patiënten met therapieresistent astma en bij 50% van de kinderen met recidiverende pneumonieën. Complicaties waren zeldzaam en mild; ernstige complicaties werden niet gezien. Bij kinderen met therapieresistent astma of recidiverende pneumonieën is bronchoscopie een nuttige diagnostische procedure, die veilig in een algemeen ziekenhuis kan worden toegepast door een ervaren bronchoscopist en onder algehele anesthesie met bewaking van vitale functies.
