Summary
Early diagnosis and treatment of bone and joint infections will prevent severe complications. S. aureus is the most common causative organism, in specific conditions, other micro-organisms may be found. The most important symptoms include high fever, malaise, local pressure pain and impaired function of the involved extremity. In case of arthritis, hydrops may develop. Radiographic procedures such as ultrasonography and magnetic resonance imaging (mri) are helpful in making the diagnosis. Infectious parameters, like C reactive protein (crp) and the erythrocyte sedimentation rate (esr) are helpful in follow-up of the disease during treatment.
The most certain diagnosis is obtained by microbiology. Culture of a biopsy specimen from the metaphysis or joint gives the highest probability of isolating the pathogen. Antibiotic treatment is focussed towards the most likely pathogens. In some circumstances, surgical treatment is necessary, especially when focal or chronic osteomyelitis is present, or when arthritis is part of the inflammatory process. Collaboration between paediatrician and orthopaedic surgeon is essential to achieve the best treatment of the patient.

Tijdige diagnose van bot- en gewrichtsinfecties leidt tot preventie van ernstige complicaties. Meestal is S. aureus de verwekker; in specifieke omstandigheden worden ook andere verwekkers aangetroffen. Belangrijkste symptomen zijn hoge koorts, malaise, lokale drukpijn en functiebeperking van de aangedane extremiteit. Bij artritis zal zich snel hydrops ontwikkelen. Beeldvormende diagnostiek zoals echografie en kernspinresonantietomografie (mri) is behulpzaam in het vaststellen van de diagnose. Infectieuze parameters, als C-reactieve proteïne (crp) en bezinking, helpen vooral bij het vervolgen van de ziekteactiviteit tijdens behandeling.
De meest zekere diagnose wordt verkregen door microbiologische diagnostiek. Biopsiemateriaal vanuit aangedane metafyse of gewricht geeft de grootste kans op het isoleren van de verwekker. Antibiotische behandeling is gericht op de meest waarschijnlijke verwekker. In een aantal gevallen is chirurgische behandeling noodzakelijk, vooral bij vormen van focale en chronische osteomyelitis en wanneer een gewricht in het ontstekingsproces is betrokken. Samenwerking tussen orthopeed en kinderarts is essentieel voor een optimale behandeling van de patiënt.
