De rol van micro-organismen bij de ontwikkeling van allergie, inzichten en mogelijkheden van preventie

Samenvatting

Micro-organismen, met name virussen, kunnen op diverse wijze een rol spelen bij het verloop van allergische aandoeningen. Een aantal epidemiologische waarnemingen suggereert dat een relatief hoge frequentie van respiratoire infecties samengaat met een relatief lage prevalentie van allergie en astma. Leefomstandigheden die samengaan met relatief veel luchtweginfecties (grote gezinnen, meerdere oudere kinderen, vroeg crèchebezoek) zijn geassocieerd met minder voorkomen van astma. Hierop is het concept gebaseerd dat infecties beschermend kunnen werken tegen allergische aandoeningen. Daarentegen blijkt dat respiratoire infecties een veel voorkomende ( 80%) trigger vormen van astmatische exacerbaties, met name bij jonge kinderen, hetgeen gepaard gaat met de inductie van bronchiale hyperreactiviteit en ontstekingsfenomenen.
