Vragen rond de behandeling van hyperactiviteit met methylfenidaat

Dit artikel geeft een inleiding in de wetenschappelijke literatuur over de behandeling van hyperactiviteit met methylfenidaat (merknaam Ritalin). De effecten van dit medicijn blijken sterk individueel en gevarieerd. Bovendien zijn er enkele onopgeloste methodologische problemen. Een gevalsstudie bij een residentieel behandelde hyperactieve jongen illustreert de onzekerheden. Er wordt een aantal kwesties besproken die bij het voorschrijven van methylfenidaat twijfel wekken. Geconcludeerd wordt dat, ook al worden in groepsstudies effecten gevonden, het individueel voorschrijven van methylfenidaat enige scepsis verdient en dat effect–evaluatie direct gekoppeld zou moeten worden aan van te voren gespecificeerde doelstellingen.

Summary

This paper gives a scientific background to a number of aspects of the use of methylphenidate in hyperactivity. The results show that the effects are highly individual and varied. Some methodological problems remain unsolved. It illustrates the uncertainties with a case study. The conclusions are, in clinical practice, that the use of methylphenidate may be justified, but that objectives should be specified and effect evaluation should be carried out on the child's behaviour immediately.
