Behandeling van angstige kinderen: twee casusbeschrijvingen

Samenvatting

Dit artikel begint met een bespreking van enkele onderzoeksresultaten over etiologie en behandeling van angststoornissen bij kinderen. Vervolgens wordt de behandeling van twee kinderen met een angststoornis beschreven. Bij Arthur, een kind met separatieangst, bestond de behandeling deels uit individuele gesprekken, waarin hij met behulp van exposureopdrachten en ‘helpende trucs’ leerde om geleidelijk meer dingen alleen te doen. Daarnaast werden met zijn ouders enkele gesprekken gevoerd, waarin zij tips kregen om Arthur bij het oefenen te helpen.

Bij Anne, een meisje met agorafobie, bleken individuele gesprekken niet haalbaar, omdat zij dat helemaal niet nodig vond. In haar behandeling lag de nadruk op gesprekken met de ouders, die in deze gesprekken vooral leerden om wat nuchterder en steviger op Anne te reageren en wat minder angstig te zijn dat Anne nare gevoelens opkropte. In de beschouwing wordt ingegaan op indicaties om de ouders meer of minder intensief in de behandeling te betrekken.

Abstract

This contribution starts with some remarks on research on etiology and treatment of anxiety disorders in children. Furthermore, the treatment of two children with an anxiety disorder is described. For Arthur, a child with separation anxiety disorder, the treatment partly consisted of individual sessions, in which he gradually learned to do more activities on his own, by means of exposure in vivo assignments and coping strategies. Also, some sessions were held with the parents in which they got tips for helping Arthur to practice. With Anne, a girl with agoraphobia, individual sessions did not seem possible, because she did not find them necessary. In her treatment, the emphasis was on sessions with the parents, who mainly learned to react more firmly on Anne, and to be less anxious that she would suppress all kinds of unpleasant feelings. Finally, indications for involving parents more or less intensively in the treatment are outlined.
