Samenvatting
Er wordt verslag gedaan van de effecten van een geprotocolleerde breedspectrum behandeling van boulimia nervosa en van de evaluatie van de verschillende onderdelen door patiënten van een psychiatrische polikliniek. Maatregelen ter vergroting van zelfcontrole, cognitieve interventies en exposure werden pas genomen nadat de patiënten in de tweede zitting hadden toegestemd geen compensatiemaatregelen meer toe te passen. Al vroeg in de behandeling van tien zittingen bleken alle onderzochte patiënten gestopt te zijn met compensatoire acties als braken en laxeren. Na afloop van de behandeling hadden negen van de tien onderzochte patiënten geen excessieve eetbuien meer. Een van hen viel in de follow-up periode van twaalf weken blijvend terug. De resultaten en de evaluaties van de patiënten achteraf ondersteunen de conclusie dat de afspraak om niet te compenseren, het stimuleren van (niet excessief) sporten, het herstellen van goede voedingsgewoonten en zelfcontrole, een cruciale rol spelen in de daling van eetbuien. Bestudering van de deelnemers die niet op de behandeling reageerden, wijst uit dat dezen aan ernstiger psychopathologie lijden en een negatiever beeld hebben van hun lichaam dan degenen die wel baat vinden bij de therapie.
Abstract
The effects of a broad spectrum cognitive-behavioral protocolled treatment of bulimia nervosa are reported. In addition to the outcome measures, the patients (out-patient treatment at a psychiatric hospital) rated the importance of the different therapeutic interventions that are included in the program. Cognitive interventions and self-control measures were only taken after the patients had agreed in the second session to stop purging after binge eating attacks. Early in the ten-session treatment all patients had stopped with purging behavior such as vomiting and taking laxatives. After treatment, nine out of the ten patients ceased having binge eating attacks. Two of them relapsed in the follow-up period of twelve weeks. The results and subjective evaluations by the patients supported the hypothesis that the agreement not to purge, the establishment of healthy food intake patterns, physical exercise (not excessive), and self-control measures were crucial. Inspecting the data of the non-responding patients suggests that they suffer from more psychopathology and even more negative views of their body than the patients who responded well to treatment.
