Een overzicht en een gevalsbeschrijving

Samenvatting

Volgens recente cijfers lijdt een kleine 10 procent van de Nederlanders aan (ernstige) slapeloosheid. De meest voorkomende therapie is nog steeds het voorschrijven van slaapmedicatie, hoofdzakelijk benzodiazepines. In de afgelopen tien jaar is goede vooruitgang geboekt op het gebied van gedragsmatige behandelwijzen van insomnie. Niet–medicamenteuze interventies sorteren meestal een betrouwbaar en duurzaam effect en zijn vaak een goed alternatief voor slaappillen. Een verbeterde diagnostiek en een verfijning van onderzoeksmethoden hebben hier toe bijgedragen. In dit artikel wordt een overzicht gegeven van de meest gestelde diagnoses bij insomnie. De hierbij passende onderzoeksmethoden en behandelwijzen (o.a. gedragstherapie, cognitieve therapie, chronotherapie, en afbouw van slaapmedicatie) worden uitgebreid beschreven. Een aantal van de procedures wordt geïllustreerd aan de hand van een gevalbeschrijving.

Abstract

According to recent figures almost 10% of the Dutch population suffers from (severe) insomnia. The majority of cases is still being treated with pharmacotherapy, benzodiazepines in particular. In the last decade much progress has been made in the development of the behavioural management of insomnia. Due to improved diagnostics and methodology the effects of non–pharmacological interventions have proven substantial and enduring, and may offer a good alternative for sleep medication. This paper reviews a number of the more prevalent diagnoses of insomnia and discusses assessment procedures and various non–pharmacological treatment modalities. Against this background a case study is reported.
