Pijn bij ouderen, een uitdaging voor de toekomst

Samenvatting

De toenemende omvang en veranderende leeftijdsopbouw van de Nederlandse bevolking zal in de komende decennia leiden tot een toename van het aantal mensen van 65 jaar en ouder. De prevalentie van pijn is bij ouderen groter dan bij volwassenen; zij varieert van 40% in de algemene populatie tot 80% bij verpleeghuispopulaties. Uit onderzoek blijkt dat er sprake is van zowel onderdiagnostiek als onderbehandeling van pijnklachten. Dit kan te maken hebben met specifieke kenmerken van de patiënt, kennistekort bij de arts en het ontbreken van geschikte assessmentinstrumenten. De medicamenteuze behandeling van pijn bij de geriatrische patiënt wordt bemoeilijkt door veranderingen in de farmacokinetiek bij ouderen, door comorbiditeit en door comedicatie. Herkenning van de cognitieve, psychische, gedrags- en sociaal-culturele dimensies van pijn geeft aanknopingspunten voor niet-farmacologische maatregelen ter verbetering van de pijnklachten. Specifieke protocollen voor pijnbehandeling bij ouderen zijn er niet. De effectiviteit van gangbare therapieën is bij ouderen nauwelijks onderzocht.
