Biologische risicofactoren voor kwetsbaarheid

Het doel van dit onderzoek was nagaan of de serumconcentraties van endocriene en ontstekingsfactoren – 25-hydroxyvitamine D3 (25(OH)D3), interleukine 6 (il-6), C-reactief proteïne (crp) en insulineachtige groeifactor-1 (igf-1) – geassocieerd zijn met reeds bestaande kwetsbaarheid respectievelijk het ontstaan van kwetsbaarheid. Voor dit onderzoek zijn gegevens gebruikt van de Longitudinal Aging Study Amsterdam (lasa), een prospectief cohortonderzoek waarbij elke drie jaar vervolgmetingen worden verricht. De respondenten waren mannen en vrouwen van 65 jaar en ouder, die tweemaal, namelijk in 1995/1996 (T1) en in 1998/1999 (T2), hadden deelgenomen aan het onderzoek.

Kwetsbaarheid was gedefinieerd als de aanwezigheid van drie of meer kwetsbaarheidsindicatoren. Bestaande kwetsbaarheid werd gemeten op T1 en het ontstaan van kwetsbaarheid tussen T1 en T2. Op T1 waren 242 (19,0%) respondenten kwetsbaar. Deze personen hadden een verhoogde concentratiecrp en een verlaagde concentratie 25(OH)D3 in het serum. Na correctie voor mogelijke confounders wascrp niet meer geassocieerd met reeds bestaande kwetsbaarheid. De concentratie 25(OH)D3 bleef geassocieerd met al bestaande kwetsbaarheid. Van alle niet-kwetsbare respondenten waren er 125 (14,1%) kwetsbaar geworden op T2. Na correctie voor mogelijke confounders was een laag gehalte 25(OH)D3 (< 25 nmol/l) geassocieerd met het ontstaan van kwetsbaarheid. Verder was een matig verhoogde serum-crp-concentratie (3,0-10,0 μg/ml) geassocieerd met het ontstaan van kwetsbaarheid. Er werden geen consistente associaties gevonden tussen kwetsbaarheid enil-6 enigf-1.
