Acute visusdaling: oorzaken, diagnostiek en behandelingsmogelijkheden

Samenvatting

Een (sub)acute visusdaling is een vermindering van de centrale gezichtsscherpte tot 0,1 of minder, die plotseling (acuut) ontstaat of in de loop van enige uren tot dagen is ontstaan. Met een goede en gerichte anamnese en lichamelijk onderzoek is de huisarts vaak zelf in staat de oorzaak te vinden en een besluit te nemen over de snelheid van doorverwijzen. Aanwijzingen uit de anamnese over pijnlijk versus niet-pijnlijk en cerebrovasculair versus cardiovasculair zijn vaak al doorslaggevend voor de beslissing iemand direct in te sturen.

De belangrijkste oorzaken van acute visusdaling zijn vaatafsluitingen, glasvochtbloeding, ablatio retinae, acuut glaucoom en aandoeningen van de n. opticus. Bij onduidelijkheid over de oorzaak van de visusdaling dient altijd direct verwezen te worden.
