Samenvatting
Overvoeding is het grootste voedingsprobleem in Nederland, maar daarnaast komt bij ouderen en chronisch zieke patiënten ondervoeding regelmatig voor. De diagnose ondervoeding wordt meestal gesteld bij 5% gewichtsverlies in een maand tot 10% verlies in een half jaar. Het mechanisme is een lagere energie-inneming dan verbruik. Bij ouderen en zieken is de inneming dan dikwijls lager dan 1500 kcal per dag en een dusdanig laag energiegebruik gaat dikwijls gepaard met een insufficiënte voorziening van vitamines en mineralen. De definiëring, klinische relevantie, epidemiologie, de meest voorkomende voedingsdeficiënties, anamnese, diagnostiek, oorzaken en behandeling worden besproken. Het belang van een adequate behandeling wordt onderstreept door de in onderzoek aangetoonde relatie tussen enerzijds een inadequate voedingstoestand en anderzijds verminderd lichamelijk en geestelijk functioneren, verminderde kwaliteit van leven, ziekte en sterfte.
