Samenvatting
Otitis media met effusie is een meestal asymptomatisch verlopende aandoening, waarbij er gedurende een lange periode (> 3 maanden) vocht in het middenoor aanwezig is. Als er al klachten zijn liggen ze op het terrein van het gehoor, de taal- en spraakontwikkeling, de schoolprestaties of het gedrag. Om de diagnose te kunnen stellen moet de mate van beweeglijkheid van het trommelvlies en middenoorsysteem bepaald worden. Op korte termijn worden vaak effecten op de taal-spraakontwikkeling gezien. Op lange termijn zijn er in meeste gevallen maar weinig gevolgen herkenbaar. De behandeling bestaat vooral uit ‘watchful waiting’ bij kinderen uit de risicogroep. Kinderen die niet tot de risicogroep behoren, hoeven niet actief gevolgd te worden. Medicijnen zijn niet effectief. Chirurgische behandeling is in de meeste gevallen ook weinig effectief. Alleen bij risicogroepen kan men in bepaalde gevallen het plaatsen van trommelvliesbuisjes of een adenoïdectomie overwegen Bij de overweging wel of niet ingrijpen speelt de huisarts een cruciale rol.
